- If you don’t have a contract with us yet, kindly fax to us your company fax cover sheet. Make sure your company name, address, and tel# and fax# is on your fax cover sheet. We will make a contract and fax it to you along with a personal guarantee form for your review and to be signed and to be faxed back to us. Once the contract & personal guarantee form is completely signed and all requirements given, then we can start seeing your patients.
